We have created three satisfaction surveys to evaluate client satisfaction with various aspects of the treatment process. Please select and complete the survey that fits with your stage of treatment. If you would prefer to complete the survey on paper, you may request a copy from your counselor or an administrative staff member.
Admission Satisfaction Survey
Complete this survey if you have recently completed an initial Evaluation or Intake and are new to Epoch or recently re-admitted to our program. The purpose of this survey is to collect information specifically related to our evaluation and admission process.
Treatment Satisfaction Survey
Complete this survey if you are an active client at Epoch and have been in treatment for 60 days or more. This survey should be completed along with every treatment plan review during your treatment. The purpose of this survey is to collect information specifically related to your counseling experience at midpoints throughout your treatment.
Overall Satisfaction Survey
Complete this survey if you have recently completed treatment successfully at Epoch. You may also complete this survey if you left treatment for any reason before completing our program. The purpose of this survey is to collect information reflective of your overall experience after leaving Epoch.
If you are not currently a client at Epoch or would like to provide general feedback (i.e. suggestions, complaints, compliments, etc.), please send us an e-mail by selecting the link below that corresponds with the Epoch location in which your experience was related.

We review e-mails on a weekly basis and will respond to any requests for follow-up as soon as possible.
